Designated Representative Authorization
If neither the student nor the listed primary contact (parent) will be present during the scheduled pick-up or delivery appointment, you must authorize another person to act on your behalf by completing the Designated Representative Form below.
This ensures our movers can access your space, release or receive your items, and confirm inventory at the appointment.
When You Need This Form
You do not need this form if:
- The student will be present, or
- The parent/primary contact listed on the account will be present
You must complete this form if:
- A roommate, friend, RA, housing staff, or anyone else will be meeting our movers in your place
- You will be traveling, off-campus, or otherwise unavailable at your appointment time
- The person present is not listed on your Deliverbees College account
What Your Designated Representative Can Do
✅ Sign for items being picked up or delivered
✅ Confirm item count and condition
✅ Communicate with our movers on your behalf during the appointment
❌ Cannot change your order, add or remove items, or dispute charges after the appointment
❌ Cannot submit damage or missing item claims (must be done by account holder only)
Liability & Responsibility Notice
By naming a Designated Representative, you authorize that person to act in your place. Their confirmation of item count and condition will be treated the same as if you were present. Any claims must still be submitted by the account holder within 72 hours of delivery.
If no one is present and no representative is assigned, a missed appointment fee may apply.
Submission Deadline
The form must be submitted at least 24 hours before your scheduled appointment time.
